Opening Wedge Distal Femoral Osteotomy in Genu Valgum Using Puddu Plate in Children above 10 Years.

Abstract

Background
The ideal method for the correction of genu valgum deformity is controversial. Opening wedge distal femoral varus osteotomy (DFVO) is one of the widely used procedures to correct the deformity in adults. However it is not the same in children. This prospective study is to evaluate the functional and radiological results of Opening wedge distal femoral osteotomy of genu vulgum using short Puddu plate in children more than 10 years.
Patients and Methods
This is a prospective study of a consecutive series of 30 cases of persistent severe genu valgum that underwent a lateral opening wedge DFVO and fixed using short Puddu plate at Beni-Suef university hospital between June 2016 and December 2019. Patients had a mean age of 12.25±1.3 years. Mean follow-up for all patients was 18 months (range 6–30 months). Patients were evaluated clinically and radiologically. The function of the knee was assessed by Bostman's score.
Results
We observed a significant improvement in the mean intermalleolar distance from 16.5cm to 4.2cm, The mean LDFA from 78.4°±2°(range 72°–85°) to 88.8°±2.5°(range 84°–93°), mean radiological TFA from 24.8°(range 16°–32°) to 7.5°(range 5°–13°) and The mean Bostman’s score from 21.5(18–24) to 29.1(27–30).
Conclusions
Opening wedge distal femoral osteotomy of genu vulgum using short Puddu plate in children above 10 years is simple, safe, reproducible procedure with low complication rate.
