Background Various surgical techniques are utilized for optimal fixation of proximal phalanx fractures, with transarticular pinning and cross-pinning from the base being two commonly employed methods. Patients and Methods This prospective study involved 40 patients treated at Elhadra University Hospital, divided into two groups of 20 each, with extra-articular short oblique or transverse fractures of the proximal phalanx. Kirschner wire fixation was performed using either transarticular pinning of the metacarpophalangeal joint or cross pinning from the base of the proximal phalanx. The outcomes were assessed using total active range of motion and Quick-DASH scores at three and six months postoperatively. Results Both fixation methods yielded comparable total active range of motionoutcomes at 3 and 6 months, with no statistically significant difference between the groups. Improvement in range of motion was observed in all joints between the 3- and 6-month follow-ups. Similarly, Quick-DASH scores indicated no significant difference between the two groups. Conclusions Both transarticular and cross pinning techniques provide stable fixation and support effective fracture healing, allowing the choice of technique to depend on surgeon preference and technical considerations.
